My idea was to use the original 5010 PT and use that for the negative bias supply, and at the same time use it to tap a positive reference voltage for the heaters

Better is to have gut shots to rule out that the voltage selector was hardwired to 220/240

Mains fuse should indeed be increased for the US. Lower voltage means higher current for the same amount of power
